Why Your RAG Implementation Isn’t Working (And How to Fix It)

You built a RAG system. It worked great on the demo dataset. Your team was excited. Then you connected it to real data and the answers started coming back wrong. Confidently wrong. Sound familiar?

I’ve been called in to fix more RAG implementations than I’ve built from scratch. The failure modes are remarkably consistent across organizations, and most of them are fixable without rebuilding the entire system. This post walks through the most common reasons RAG systems fail in production and the specific fixes I apply.

Problem 1: Your Chunks Are Wrong

This is the single most common issue and the one with the biggest impact on answer quality. Most teams use a fixed chunk size (say, 512 or 1024 tokens) with some overlap and call it done. This works for uniform content like documentation pages but fails badly for content with varying structure.

Symptom: The system retrieves chunks that contain part of the answer but not all of it. Answers are partially correct but miss crucial context or include irrelevant information from adjacent sections.

Why it happens: Fixed-size chunking splits content at arbitrary boundaries. A paragraph explaining a configuration procedure gets split across two chunks. The first chunk has the steps, the second has the important warning about what not to do. The retriever finds the first chunk but not the second, and the generated answer tells the user how to do something without mentioning that it will break their system under certain conditions.

The fix: Switch to semantic chunking or structural chunking. Semantic chunking uses an LLM or embedding model to identify natural break points in the content, where the topic actually changes. Structural chunking uses document structure (headers, sections, paragraphs) to create chunks that represent complete thoughts.

In practice, I use a hybrid approach: chunk at structural boundaries (sections, subsections) and then split any chunks that exceed a maximum token limit at the nearest paragraph break. This preserves context while keeping chunks within the embedding model’s effective range.

Quick test: Pull 20 random chunks from your vector store and read them in isolation. Do they make sense as standalone pieces of information? If most of them start mid-sentence or end mid-thought, your chunking strategy needs work.

Problem 2: Retrieval Is Finding the Wrong Documents

Your chunks might be well-formed, but if the retriever is pulling irrelevant ones, the generated answers will still be wrong.

Symptom: The system returns answers that are topically adjacent but factually wrong for the specific question. The retrieved chunks are about the right topic but not the right aspect of it.

Why it happens: Pure vector similarity is a blunt instrument. The embedding for “how to reset my password” and “our password policy requires 12 characters” are semantically similar (both are about passwords) but they answer very different questions. The retriever cannot distinguish between these without additional signals.

The fix: Three approaches, in order of effort.

First: metadata filtering. Add metadata to your chunks (document type, product, date, category) and filter before retrieval. If the user is asking about Product A, filter out chunks from Product B documentation before the similarity search runs. This eliminates a large class of “right topic, wrong context” errors.

Second: hybrid retrieval. Combine vector similarity with keyword (BM25) search. Vector search finds semantically related chunks; keyword search finds exact term matches. The combination catches cases where the embedding misses a crucial term. Most vector databases (Weaviate, Qdrant, Pinecone) now support hybrid search natively.

Third: re-ranking. After initial retrieval, run the top 20-30 results through a cross-encoder re-ranking model that scores each chunk against the original query more carefully than the initial embedding comparison. This is computationally more expensive (you cannot re-rank the entire corpus) but dramatically improves precision in the final set of chunks passed to the LLM.

I use all three in production. The combination of metadata filtering + hybrid retrieval + cross-encoder re-ranking produces noticeably better results than any single approach.

Problem 3: The LLM Is Ignoring Retrieved Context

Sometimes the retriever does its job perfectly (the right chunks are found) but the LLM ignores them and generates an answer from its parametric memory instead.

Symptom: The generated answer sounds authoritative but does not match any of the retrieved documents. Alternatively, the answer starts by citing retrieved content and then drifts into information that came from the model’s training data.

Why it happens: LLMs have a strong prior from pre-training. If the retrieved context contradicts what the model “knows” from training, the model sometimes defaults to its training data, especially when the retrieved context is ambiguous or poorly formatted.

The fix: Prompt engineering, specifically.

Make context placement explicit. Put the retrieved context early in the prompt, wrapped in clear delimiters. Tell the model: “Answer the question using ONLY the context provided below. If the context does not contain enough information to answer the question, say so.”

Reduce context noise. Passing 15 chunks to the LLM when only 3 are relevant dilutes the signal. Be aggressive with the number of chunks you include. I typically pass 3-5 chunks for factual questions and 5-8 for more complex questions. More is not better.

Add source attribution to the prompt. Ask the model to cite which document each piece of its answer comes from. This forces the model to ground its answer in the retrieved context and makes it obvious when it drifts into parametric memory (the citation will be missing or wrong).

Problem 4: Stale or Contradictory Data

Symptom: The system gives answers that used to be correct but are no longer accurate, or gives different answers to the same question depending on which chunks are retrieved.

Why it happens: Your source data has not been re-indexed, or your source data contains contradictory information (old and new versions of the same document both exist in the corpus).

The fix: This is an operations problem, not an engineering problem.

Implement incremental updates. Your vector store must be refreshed when source documents change. Set up a pipeline that detects changes in the source system (CMS, knowledge base, document store) and re-indexes affected documents. Full re-indexing should run on a regular schedule as a consistency check.

Handle contradictions explicitly. When your corpus contains multiple versions of the truth (old and updated articles about the same topic), add metadata that identifies the authoritative version. Use recency as a ranking signal: when two chunks are equally relevant, prefer the newer one.

For organizations with large knowledge bases where contradictions are endemic, I recommend a GraphRAG approach that explicitly models supersession relationships between documents. This lets the system know that Article B replaced Article A and always prefer the current version.

Problem 5: No Evaluation Framework

This is not a technical problem in the RAG pipeline; it’s a process problem. But it causes more failed RAG projects than any single technical issue.

Symptom: The team cannot objectively answer the question “Is our RAG system good?” Changes are made based on anecdotal reports (“Someone said it gave a wrong answer last week”) rather than systematic measurement.

Why it happens: RAG evaluation requires effort to set up and most teams skip it in the rush to demo.

The fix: Build an evaluation dataset of 50-100 question-answer pairs. These should be real questions from real users (or realistic approximations) with verified correct answers. Run every RAG pipeline change against this evaluation set before deploying. Measure three things:

Retrieval precision: Do the retrieved chunks contain the information needed to answer the question?

Answer accuracy: Is the generated answer factually correct according to the ground truth?

Answer faithfulness: Is the generated answer grounded in the retrieved context (not hallucinated from parametric memory)?

Automated evaluation using an LLM as a judge works surprisingly well for answer accuracy and faithfulness. For retrieval precision, you need human-labeled ground truth chunks for at least a subset of questions.

The Order of Operations

If your RAG system is underperforming, here is the order I attack these problems.

Step 1: Build an evaluation dataset. You cannot fix what you cannot measure.

Step 2: Fix chunking. This has the highest impact-to-effort ratio.

Step 3: Add hybrid retrieval and metadata filtering. Second-highest impact.

Step 4: Tune prompts to improve context adherence. Quick wins here.

Step 5: Add re-ranking if retrieval precision is still below target.

Step 6: Implement freshness and contradiction handling for production stability.

This order is not arbitrary. It reflects the dependency chain. Better chunks improve retrieval. Better retrieval reduces the burden on prompt engineering. And none of it matters without evaluation to confirm you are actually making things better.
